Simple Practice offers an easy to use interface with convenient features for clients.  Clients can fill out prescreening forms, submit insurance/payment information, review chart notes, obtain lab information, request medication refills, schedule appointments, send secure messages to provider for any needs/concerns, and get links to upcoming appointments.  Text reminders will be sent for appointments as a convenient way to ensure making appointments.  

Feel free to submit prescreening forms and I will review to create a link to set up your Patient Portal Account.   Assistance is available on the Simple Practice site.  Once you enter your insurance information or provide a copy of your insurance card I will be able to perform an eligibility check for your insurance plan prior to appointment if using insurance.
